To submit a referral the Fisher House must receive a completed Referral Form and DD Form 2870 (click on names to download). Instructions can be downloaded here. Completed forms may be faxed (253-968-3619) or e-mailed (fhmamc@aol.com) to Fisher House staff. All referrals must be completed by 1300 (PST), Monday – Friday, for same day placement. If not for same day placement, please allow one to two business days for a staff member to contact you.
Who can submit a referral
If patient is inpatient and an active duty service member: see service member’s case manager.
If patient is inpatient and a dependent of a service member: see social work.
If patient is outpatient: please contact Fisher House staff for appropriate contact
If the Fisher House is full, there is a chance that the family may qualify for off-site lodging assistance. The Fisher House Foundation is currently providing funding for off- site lodging for families that qualify for the Fort Lewis Fisher House and that are not on Military Travel Orders of any kind. Funding will not be provided without the required referral information (DD Form 2870 and FH Referral Form) that determines eligibility. Family must be placed on waiting list and be willing to move to the FH when a room becomes available, otherwise the funding will terminate at that time.
Off site lodging must be coordinated before 1:00 pm PST, Monday – Friday.
